ABSTRACT

OBJECTIVE To analyze the influential factors of cardioto xicity in patients with positive breast cancer of human epidermal growth factor receptor 2(HER-2)treated by trastuzumab combined with chemotherapy. METHODS From April 2017 to January 2021,200 HER-2 positive breast cancer patients receiving pirarubicin + cyclophosphamide combined with sequential paclitaxel+trastuzumab were collected from our hospital. According to the presence or absence of cardiotoxicity ,the patients were divided into cardiotoxicity group and non-cardiotoxicity group. The clinical data and echocardiographic results of the patients were collected,and the influential factors of cardiotoxicity were analyzed. RESULTS Among 200 patients,43 patients suffered from cardiotoxicity with the incidence of 21.5%. The proportion of patients with cardiotoxicity during pirarubicin+cyclophosphamide therapy accounted for 5.5%(11/200),and the proportion of patients with cardiotoxicity during sequential paclitaxel+trastuzumab therapy accounted for 20.5%(41/200);the latter was significantly higher than the former (P<0.01). At the same time ,the decrease of left ventricular ejection fraction during sequential therapy of paclitaxel and trastuzumab was significantly higher than that during pirarubicin+cyclophosphamide therapy [ 14%(12%,17%)vs. 7%(3%,10%),P<0.001]. Compared with patients without cardiotoxicity ,the proportion of patients with cardiotoxicity with a history of hyperlipidemia was significantly higher (P< 0.01),while the proportion of patients receiving dexrazoxane was significantly lower (P<0.01). Results of binary Logistic regression analysis showed that the history of hyperlipidemia [OR =3.672,95% CI(1.499,8.992),P=0.004] and the use of dextrazoxane [OR =0.154,95% CI(0.072,0.330), P<0.001] were associated with the occurrence of cardiotoxicity. CONCLUSIONS Hyperlipidemia is an independent risk factor for cardiotoxicity induced by pirarubicin + cyclophosphamide combined with sequential paclitaxel+trastuzumab in HER 2 positive breast cancer patients ,while the use of dextrazoxane is a protective factor.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the changes in the mechanical properties of the right ventricular myocardium (RVM) after chemotherapy using three-dimensional speckle tracking echocardiography (3D-STI).</p><p><b>METHODS</b>Thirty-six breast cancer patients receiving chemotherapy with pirarubicin underwent examinations with 3D-STI to test the mechanical properties of the RVM before chemotherapy and at the end of the second, fourth, and sixth cycles of chemotherapy (C2, C4, and C6, respectively). Blood levels of hs-cTnI and NT-proBNP were also examined at the same time points. Thirty-one of these patients also underwent Tc-MIBI and F-FDG myocardial perfusion/metabolism imaging at C6. Myocardial perfusion abnormalities and survival outcomes of the patients were analyzed according to radionuclide imaging results.</p><p><b>RESULTS</b>Compared with that before chemotherapy, RVGLS at C2 was significantly lowered, and both RVGLS and RVGAS were significantly decreased at C4 and further decreased at C6 ( < 0.05) in relation with the cumulative drug dose. The RVGLS and RVGAS differed significantly among patients with different levels of TAPSE, hs-cTnI, and RV-FAC decline after chemotherapy. RVGLS and RVGAS were found to significantly correlate with FAC (=0.37, 0.26), TAPSE (=0.43, 0.51), and S' (=0.21, 0.36) ( < 0.01), and showed a high sensitivity and specificity for identifying RV-FAC decline by > 5%. Myocardial perfusion/metabolic imaging showed normal myocardial perfusion in 17 patients, and abnormal myocardial segments of the RVM were detected in 14 patients, but F-FDG imaging showed that these myocardial segments were all viable; these 14 patients showed significantly decreased RVGLS and RVGAS and significantly increased hs-cTnI level compared with the patients with normal ventricular myocardial perfusion ( < 0.05). ROC curve analysis showed that an absolute value of RVGLS less than 18.2% had a sensitivity of 92.9% for diagnosis of RV impairment with a diagnostic specificity of 88.2% and an area under the curve of 0.87. RVGAS less than 26.8% had a sensitivity of 94.8% and a specificity of 86.6% for diagnosis of RV damage with an area under the curve of 0.86.</p><p><b>CONCLUSIONS</b>3D-STI can provide a reliable new approach to early diagnosis of changes in the mechanical properties of the RVM related with chemotherapy with pirarubicin in breast cancer patients.</p>

ABSTRACT

Objective To evaluate the clinical cardiovascular reverse reactions of the patients with breast cancer who received chemotherapy with anthracyclines. Methods Two hundred and ninety-one cases of breast cancer who were treated with pirarubicin (40 mg/m2) + docetaxel (75 mg/m2) in Shanxi Provincial Cancer Hospital from January 2014 to December 2016 were collected. The changes of electrocardiogram (ECG), echocardiography and myocardial enzyme were recorded before and after chemotherapy. Results Before chemotherapy, there were 14 patients (4.8 %) with ST-T segment changes before ECG, 6 patients (2.1 %)with atrial extrasystole,8 patients(2.7 %)with ventricular extrasystole.After chemotherapy,cases with abnormal ECG were 62 (21.4 %), 47 (16.2 %) and 52 (17.9 %) respectively. The difference between the two groups was statistically significant (χ2= 6.249, P < 0.005). The average left ventricular ejection fraction after chemotherapy was significantly lower than that before chemotherapy (0.72±0.10 vs. 0.81±0.18, t= 3.480, P <0.05). The levels of creatine kinase (CK), aspartate aminotransferase (AST) and creatine kinase CK (CK-MB) were (68±20), (35±13) and (15.1±4.0) U/L after chemotherapy respectively, which were significantly higher than those before chemotherapy [(50 ±12), (28 ±12) and (7.9 ±2.6) U/L], the differences were statistically significant (t values were 9.721, 5.137 and 18.850 respectively, all P < 0.005). Conclusion The use of anthracyclines results in the increased risk of ECG abnormalities and myocardial damage in patients with breast cancer,and cardiac dynamic monitoring should be noted during chemotherapy.

ABSTRACT

Objective To investigate the nursing intervention and efficacy of docetaxel combined with pirarubicin in the treatment of advanced breast cancer. Methods In our hospital from January 2014 to February 2015 were selected from the patients with advanced breast cancer in 94 cases as the research object, the single and double number (hospital admission order) of the selected patients with advanced breast cancer were randomly divided into study group and control group, 41 cases in each group. 2 groups of patients with advanced breast cancer were treated with docetaxel combined with pirarubicin, the control group was given routine nursing services, and the study group was given comprehensive nursing intervention measures on the basis of routine care. Results Of the 2 groups of patients with advanced breast cancer nursing after different nursing measures the same drug plan, after the study group, the total efficiency of treatment in the method [please explain what is near total efficiency] (82.98%) was significantly better than the control group (61.70%), the difference was statistically significant (P<0.05); the study group the nursing service satisfaction was significantly better than the control group (P<0.05). Conclusion With the combination of docetaxel and pirarubicin in the treatment of advanced breast cancer patients, comprehensive nursing measures can achieve better clinical efficacy and benefit the maintenance of good relationship between nurses and patients.

ABSTRACT

Objective To evaluate the efficacy of preoperative and postoperative intravesical instillation of pirarubicin for the prevention of postoperative recurrence of non-muscle invasive bladder cancer (NMIBC). Methods From September 2012 to March 2014, 120 patients who were diagnosed as NMIBC and accepted trans-urethral resection of bladder tumor (TUR-Bt) were collected and randomly divided into two groups, including treatment group (60 cases) and control group (60 cases). 30 minutes before the operation, instillation of pirarubicin (40 mg) was performed in the treatment group, then pirarubicin perfusion was given within 24 h after operation, once a week for 8 weeks, and once a month for 10 months after operation. In control group, instillation of pirarubicin (40 mg) was given within 24 h after operation, and other treatments were the same as the treatment group. The differences of lesion detection probability, the 2-year recurrent rate and the adverse reaction rate were compared between the two groups. Results The lesion detection rates were 42.70 % (38/89) and 1.85 % (2/108) in the treatment group and control group, respectively (χ2=50.303, P 0.05]. Conclusions The preoperational intravesical instillation of pirarubicin can fix and diagnose malignant lesions in the bladder without adverse reactions, which will improve the rate of tumor resection and prevent postoperative recurrence of tumors.

Methods 86 patients from August 2015 to December 2016 for transurethral resection of bladder tumor treated with pirarubicin(PHT) early intravesical therapy were divided randomly into experimental group 43 cases and control group 43 cases, control group in transurethral resection of bladder tumor after transurethral resection take the routine drug regimen, early implementation of pirarubicin therapy after surgery in the experimental group, to compare and analyze the recurrence of bladder cancer after operation in the two groups, and two groups of patients were given psychological nursing, to explore the implementation and effect of psychological nursing measures. Results The patients in the experimental group the recurrence rate was 9.3%, significantly higher than the control group 18.6%, the differences between groups was statistically significant (P<0.05); two groups of patients were given psychological nursing was found to improve patient psychological treatment, increase treatment compliance. Conclusion THP in transurethral resection of bladder tumor after star early bladder irrigation can effectively reduce the recurrence rate of bladder cancer after operation, psychological nursing in the treatment process, can enhance the tolerance in the treatment of patients, improve the treatment compliance, is worthy of clinical popularization and application.

ABSTRACT

OBJECTIVE: To assess the risk of acute cardiactoxicity associated with anthracyclines use in patients with breast cancer and investigate the influencing factors on acute cardiactoxicity. METHODS: Cross-sectional analysis was carried out for 213 inpatients with breast cancer who received chemotherapy. RESULTS: ECG changes occurred in 37% patients. It was estimated that the incidence of acute cardiactoxicity associated with anthracyclines was about 30%. The main risk factors of acute cardiactoxicity were previous chemotherapy or radiotherapy, relative higher age(≥55), and menopause. CONCLUSION: ECG is an effective examination method for the patients who have acute cardiactoxicity reactions associated with anthracyclines. Clinical pharmacists should give more pharmaceutical care to post-menopause patients who have previously received chemotherapy or radiotherapy and are relatively old(≥55 years).

Objective:To establish the test method for histamine-like substances in pirarubicin. Methods:According to the meth-od described in the appendix of Chinese Pharmacopoeia (2010 edition), the test for histamine-like substances was carried out. Re-sults:The test for histamine-like substances had no interference under the condition with pirarubicin concentration of 0. 01mg·ml-1 . The established test method for histamine-like substances in pirarubicin was applied in 10 samples, and the results were accordant with those of the test method for depressor substances. Conclusion:The test method for histamine-like substances in pirarubicin is feasible with the limit of 1 μg·mg-1 .

ABSTRACT

OBJECTIVE:To observe the efficacy and safety of sequential chemotherapy on the basis of 5-FU combined with three-dimensional conformal intensity modulated radiotherapy in the treatment of postoperative advanced gastric cancer. METH-ODS:110 patients with advanced gastric cancer were randomly divided into control group(55 cases)and sequential group(55 cas-es). Control group was given 400 mg/(m2·d) 5-FU injection by intravenous injection,then given 600 mg/m2 for continuous infu-sion 22 h,d1-2+50 mg/d Pirarubicin hydrochloride for injection,intravenous injection,d1+150 mg/d Oxaliplatin for injection,intra-venous injection,d1,three-dimensional conformal intensity modulated radiotherapy was conducted for 2 weeks after the 4th chemo-therapy,and the total dosage was DT40-45 Gy/1.8 Gy/24-25F. sequential group was additionally given three-dimensional conformal intensity modulated radiotherapy;3-week was regarded as a treatment course,and it lasted 6 courses. The total survival rate and progression-free survival rate in 1,2 and 3 years,metastasis rate and incidence of adverse reactions in 2 groups were followed-up. RESULTS:The total survival rate and progression-free survival rate in 1,2 and 3 years in sequential group were significantly high-er than control group,lymphatic metastasis rate and distant metastasis rate in sequential group were significantly lower than control group,the differences were statistically significant (P0.05). CONCLUSIONS:Sequential chemotherapy on the basis of 5-FU combined with three-dimensional conformal intensity modulated radiotherapy can effectively prolong the survival time and reduce long-term metastasis rate of patients in the treatment of postoperative advanced gastric cancer,with similar safety.

ABSTRACT

Objective To explore the efficacy and adverse reactions of improved VTD regimen (pirarubicin combined with vincristine and dexamethasone) plus low-dose thalidomide in patients of newly diagnosed multiple myeloma(MM).Methods Twenty-nine cases of newly diagnosed MM were enrolled in this study.The improved VTD regimen was intravenous injection vincristine 2 mg/d on the first day,intravenous drip pirarubicin 20-30 mg/d from the first day to the second day,and intravenous drip dexamethasone 8 mg/d from the first day to the tenth day.Twenty-eight days was one course of treatment.Response and adverse reactions were evaluated after 4 course of treatment.On the first day of chemotherapy,all the patients were orally administered thalidomide 50 mg/d.Three days later,thalidomide was added to 100 mg/d and chronically maintained if toxicities could be tolerated.Results There were 3 cases(10.3%) in complete response,12 cases (41.2%) in very good partial response,10 cases (34.5%) in partial response,3 cases (10.3%) in stable disease,and 1 case(3.5%) in progressive disease.The overall response rate was 86.2%.Main adverse reactions were myelosuppression,asthenia and constipation,all could be tolerated.Conclusion It has significant response rate and less side effects of improved VTD regimen plus low-dose thalidomide for the patients of newly diagnosed multiple myeloma,and deserves further clinical practice.

ABSTRACT

Objective To investigate the clinical efficacy of neoadjuvant chemotherapy regimens of TA or AC in the treatment of locally advanced triple negative breast cancer. Methods Data of 99 women with stageⅡ/ Ⅲ locally advanced triple negative breast cancer treated in the Centre Hospital of Cangzhou from Jan. 2006 to Dec. 2011 were retrospectively analyzed. These patients were divided into two groups based on the regimen of the neoadjuvant chemotherapy. Fifty-two cases were received TA regimen(Docetaxel 75 mg/ m2 and pirarubicin 50 mg/ m2 )and 47 cases were received AC( pirarubicin 50 mg/ m2 ,cyclophosphamide 600 mg/ m2 ). IV drip infusion was administered in both groups for 4 cycles before surgery,with 3 weeks for each cycle. The efficacy after treatment,the 2 year recurrence rate and overall survival rate after operation were compared between the two groups. Results The response rates in TA group were 88. 46% ,higher than that of AC group(57. 45% ),and the difference was statistically significant(χ2 = 12. 260,P < 0. 001). Furthermore,the rate of pathological grade 4 and 5 in TA group were 42. 3%(22 / 52)was superior to AC group(23. 4%(11 / 47);P = 0. 046). The 2-year recurrence rate and survival rate in TA group were 23. 08%(12 / 52)and 84. 62% ,as same as that in AC group ((27. 66%(13 / 47)and 80. 85% ;χ2 = 0. 400;P = 0. 53). Conclusion TA and AC are both effective in terms of females with stage Ⅱ/ Ⅲ locally advanced triple negative breast cancer treat with neoadjuvant chemotherapy. Moreover,TA is superior to AC. However,there is no statistical difference of 2-years recurrence rate and survival rate between two groups.

ABSTRACT

Objective To explore the efficacy of percutaneous intratumoral injection of mixed emulsion with hot lipiodol and pira-rubicin for treatment of liver metastases lack of blood supply.Methods 12 patients with liver metastases,confirmed by the clinical and pathological examination,were treated by intratumoral injection of mixed emulsion with heating lipiodol and pirarubicin using 21G biliary needle under CT guidance.The adverse events were recorded and the treatment efficacy was also assessed according to the imaging findings.Results 8 patients achieved a partial remission of lesions,and 3 ones had stable condition.No serious adverse reactions occurred.Conclusion It is effective,simple,convenient and safe for treatment of liver metastases lack of blood supply with percutaneous intratumoral injection of emulsion with hot lipiodol and pirarubicin.

ABSTRACT

Objective To evaluate the clinical efficacy and safety of transcatheter arterial chemoembolization (TACE) with oxaliplatin (OXA), fluorouracil (5-Fu) and pirarubicin (THP) scheme in treating primary liver cancer. Methods A total of 65 patients with primary liver cancer were treated with TACE using infusion of OXA/5-Fu/THP scheme (TACE group). Other 21 patients with primary liver cancer, who were encountered at the Department of Interventional Radiology of authors’ hospital during the same period as the patients of TACE group, received simple transarterial embolization (TAE group). The therapeutic effect, the occurrence of adverse reactions, the progression-free survival time (PFS) and overall survival time (OS) of the two groups were comprehensively evaluated. The results were compared between the two groups. Results In the TACE group, the objective response rate (ORR) and disease control rate (DCR) were 55.4%and 81.5%respectively, and the median PFS and median OS were 11.5 months and 18.5 months respectively. Single factor analysis indicated that patients, who had liver function of Child-Pugh A and received more times or treatment, and who had small-sized tumor and no portal vein tumor thrombus or metastasis, usually had a better prognosis, and the differences were statistically significant (P<0.05). The prognosis of the patients with Barcelona staging (BCLC) B was better than that of the patients with Barcelona staging C, and the difference was statistically significant (P=0.000). Cox multivariate analysis revealed that the portal vein tumor thrombus and tumor metastasis were independent risk factors for the prognosis of the patients. Compared with the TAE group, TACE with OXA/5-Fu/THP scheme could effectively improve the mean progression-free survival time. Conclusion For the treatment of primary liver cancer, TACE with infusion of OXA/5-Fu/THP is clinically effective with fewer adverse reactions.

ABSTRACT

Objective To investigate the effects and toxicity of pirarubicin and chemotherapy regimen in treatment of malignant lymphoma in the elderly. Methods 50 cases(over 60 years of age) with non Hodgkin's lymphoma collected in Lishui Central Hospital of Zhejiang province from January 2011 to January 2013 were randomly divided into control group(n=25)and experimental group(n=25). Experimental group were received CTOP chemotherapy, control group were received the standard CHOP chemotherapy, vomiting, stomach, blood pressure, blood glucose control and other appropriate symptomatic treatment were adopted according to the speciifc circumstances of each patient in both two groups. Regular inspection were given to all patients, and bone marrow suppression, treatment of lymphoma, complications and other indicators were observed. Results Complete remission in experimental group after treatment was 13 cases, accounted for 52%within group, partial remission were 9 cases, accounted for 36%. The total effective number was 22, and the rate was 88%. Complete remission in control group after treatment was 8 cases, accounted for 32%within group, partial remission were 5 cases, accounted for 20%. The total effective number was 13 cases, and the rate was 52%. Bone marrow suppression in experimental group was signiifcantly less than control group (P<0.05). Conclusion Elderly patients with malignant lymphoma in chemotherapy plus pirarubicin can improve the therapeutic effect, reduce bone marrow suppression and complications.

Objective To investigate the clinical effect of pirarubicin combined with cyclophosphamide chemotherapy in the treatment of non Hodgkin's lymphoma.Methods 70 cases with non Hodgkin's lymphoma were randomly divided into the observation group and control group,35 cases in each group.The observation group was treated with pirarubicin chemotherapy,the control group received doxorubicin chemotherapy scheme.The clinical efficacy and side effects of the two groups were observed.Results The effective rate of the observation group was 74.3%,which was significantly higher than 51.4% of the control group (x2 =6.28,P < 0.05).The bone marrow suppression,nausea,vomiting,abnormal liver function and other side effects between the two groups had no significant differences (x2 =1.62,0.52,0.00,all P > 0.05).The incidence rates of alopecia and ECG abnormality in the observation group were 31.4% and 11.4%,which were significantly lower than 100.0%,34.3% of the control group (x2 =36.52,5.19,all P < 0.05).Conclusion The clinical efficacy of pirarubicin combined with cyclophosphamide chemotherapy is satisfactory,its main advantage is obviously decreased heart toxicity and alopecia.

ABSTRACT

Objective To explore the effects of trimetazidine on myocardial free radical inj ury induced by pirarubicin,and to clarify the protective effect and mechanism of trimetazidine on myocardial inj ury induced by pirarubicin.Methods 3 6 Wistar rats were randomly divided into pirarubicin group (n= 1 3 ), trimetazidine intervention group(n=13)and control group (n=10).The rats in pirarubicin group and trimetazidine intervention group were inj ected with pirarubicin 2.5 mg · kg-1 by the vena caudal once a week for 6 weeks. The rats in trimetazidine intervention group were intragastricly infused with trimetazidine 5.4 mg · kg-1 · d-1 one day for 8 weeks before making the model. At the end of the experiment,the malonaldehyde (MDA)level,nitrogen oxide (NO)level,superoxide dismutase(SOD)activity,and nonprotein sulfhydryl (NPSH)level in myocardium tissue were measured. The histological changes of myocardium tissue were detected by electron microscope. Results Compared with control group ,the levels of MDA and NO in pirarubicin group were increased(P<0.05), and the SOD activity and NPSH level in pirarubicin group were decreased(P<0.05).Compared with pirarubicin group,the levels of MDA and NO in trimetazidine intervention group were decreased(P<0.05),the SOD activity and NPSH level in trimetazidine intervention group were increased(P<0.05).Under electron microscope,the myocardiocytes of the rats in pirarubicin group showed irregular arrangement in sacromere structure, shrinkage in nuclear membrane, vacuolation in nuclear matrix, obvious mitochondria swelling, deposition of metachromatin throughout the nucleus,and an indistinct view of intercalated disc with isolation;while in trimetazidine intervention group the nucleus was round and nuclear membrane was indented,myofilament bundles were decreased slightly with a regular arrangement, intercalated disc oriented transversely with partial vague in cell j unction structure, and mitochondria slightly swelled.Conclusion Trimetazidine has the protective effects on the damaged myocardiocytes caused by pirarubicin,and its mechanism may be related to reducing the production of free radicals and decreasing the injury of structures within the cells,such as the nucleus,mitochondria and intercalated disc.

ABSTRACT

Objective: To investigate the effects of pirarubicin on proliferation of bladder cancer cells and the related mechanism. Methods: After bladder cancer cell lines T24 and BIU-87 were treated with 0.4, 0.8, 1.6, and 3.2 mg/L pirarubicin for 24, 48, and 72 h, the cell proliferation was detected by MTT. Flow cytometry was used to examine the apoptosis of T24 and BIU-87 cells. qRT-PCR and RT-PCR were used to examine the mRNA expression of phospholipase C ε (PLCε),Bcl-2 in T24 and BIU-87 cell lines; the protein expression of PLCε in pirarubicin-treated cells was determined by Western blotting analysis. Bladder cancer cells were designed as blank group, pirarubicin treatment group, Ad-shPLCε treatment group, and Ad-shPLCε plus pirarubicin treatment group; cell proliferation was observed and protein expression of Bcl-2 was examined and compared between different groups. Results: Pirarubicin showed a dose-and time-dependent inhibitory effect against proliferation of T24 and BIU-87 cell lines. Moreover, pirarubicin promoted cell apoptosis in T24 and BIU-87 cells and suppressed the expression of PLCε and Bcl-2. Pirarubicin treatment group, Ad-shPLCε treatment group, and Ad-shPLCε plus pirarubicin treatment group all had suppressed cell proliferation and Bcl-2 expression, and pirarubicin plus Ad-shPLCε group exhibited significantly stronger inhibitory effects compared with pirarubicin treatment group (P<0.05). Conclusion: Pirarubicin can effectively inhibit cell proliferation of bladder cancer cells, which may be through suppressing the expression of PLCε and Bcl-2.

ABSTRACT

Objective To explore the effects of matrine on apoptosis and nuclear factor(NF)-κBp65 activity of HL-60 cells induced by pirarubicin (THP).Methods Effects of the apoptosis:the HL-60 cells in blank control group were cultured 14 hours with RPMI 1640; groups of different concentration drugs:matrine:0.1 μmol/L,1.0 μmol/L,10.0 μmoL/L and THP:1.0 μmol/L,10.0 μmol/L,100.0 μmol/L.The apoptosis of HL-60 cells were tested by fluorescence double colour painting and flow cytometry (FCM).The NF-κBp65 activity of HL-60 cells were determined by FCM.Results The rates of apoptosis of HL-60 cells were (7.14 ± 2.95) %,(12.34 ± 2.55) %,(19.3 ±2.31)% and (31.78 ±4.31)%,(47.25 ±5.27)%,(56.49 ±1.59)% in matrine 0.1 μmol/L,1.0 μmol/L,10.0 μmol/L and THP 1.0 μmoL/L,10.0 μmol/L,100.0 μmol/L groups,compared to blank control group (6.46 ± 1.45) %,there were significant difference (F =257.72,677.19 ; all P < 0.001) ; (70.17 ± 5.68) % in matrine 0.1 μ mol/L +THP 1.0 μ moL/L group[vs the THP 1.0 μmoL/L group (31.78 ±4.31)%,(t =38.94,P<0.001)].The rates of NF-κBp65 activity of HL-60 cells in matrine 0.1 μmol/L,1.0 μmol/L,10.0 μmol/L and THP 1.0 μmol/L,10.0 μmol/L,100.0 μmol/L groups were(8.34 ± 1.52)%,(7.11 ± 1.29)%,(4.78 ±0.31)% and (16.21 ± 1.20) %,(23.98 ± 3.21) %,(32.44 ± 2.89) %,with the control group (8.44 ± 2.20) %,there were significant difference(F =65.35,P < 0.001 ; F =674.11,P < 0.001) ; (12.01 ± 2.27) % in matrine 0.1 μmol/L + THP 1.0 μmol/L group [with (16.21 ± 1.20) % of THP 1.0 μmol/L group(t =21.42,P < 0.001)].Conclusions The apoptosis of HL-60 cell is induced by matrine and pirarubicin in a dose-dependent manner.NF-κBp65 activity of HL-60 cell is inhibited by matrine and increased by pirarubicin in a dose-dependent manner.Matrine can enhannce the effect of induction by pirarubicin of apoptosis of HL-60 cell,and decrease activity of NF-κBp65 by pirarubicin.

ABSTRACT

Objective To compare the efficacy and safety of intravesical instillation of pirarubicin (T HP) to prevent the post -operative recurrence of superficial bladder cancer with same dose variable concentration .Methods A tota1 of 56 cases of superficial bladder cancer after transurethral resection (TURBT ) were randomly divided into two groups(High concentration group :25 cases , Low concentration group :31 cases) .Infusion dose of THP is 30 mg .The concentration of THP is 1 .0 g/L in H group ,0 .6 g/L in L group .Both groups were installed within 24 hours after operation .Both groups were installed once a week for 8 weeks after the first installation ,then once a month until 1 year after operation .All cases were followed up ,the tumor recurrence time ,recurrence rates and complications were compared .Results All cases except 2 in L group received 6 -30 months follow up ,with an average time of 17 .6 months .In H group ,1 patients recurred during the follow-up period ,with a recurrence rate of 4 .0% and recurrence time of 12 .4 months ,while in L group ,3 cases recurred ,with a recurrence rate of 10 .3% and an average recurrence time of 9 .8 months .Adverse reactions were main1y Urinary irritation symptoms .The difference of recurrence rate and average recurrence time in each group were statistically significant (P0 .05) .Conclusion This study shows that high concentration THP intravesical instillation could effectively reduce the recur-rence rate and delay the recurrence time .Each concentration was well tolerated .
